Dear Lauren,
I am writing on behalf of T. E.'s family. T. E. is
a 15-year-old young man who was diagnosed with Neuroblastoma in 2003.
T. E. relapsed despite several
rounds of chemotherapy and radiation treatments.
T. E. lives at home with his parents and six siblings.
T. E., with the support of his parents, has decided to discontinue additional chemotherapy treatments and focus on comfort.
He is at home with his family, friends, and hospice.
I am requesting financial assistance in the amound
to $96.67 for prescription medicine that was not covered by health insurance.
