Dear Lauren,
I am writing on behalf of R. B.'s
family. R. B. is a 6-month old baby girl with multiple congenital anomalies, including Teratology of Fallot and Miller-Dieker
Syndrome. Because of her complex medical problems, she requires constant supervision for airway management, increased
congestion, and reflux and seizure activity.
R. B.'s parents have made the loving decision to not pursue aggressive
medical treatments for R. B. and focus on managing distressing symptoms and keeping her comfortable. R. B. spends her
days surrounded by her older sisters and brother at home. R. B.'s mother stopped working full-time when she was
born. Her father works full-time to support the family. I'm writing to request financial assistance in the
amount of $3,611.42 to cover the cost of household bills.
